Non-emergency allergy prep (24h)
  • use for allergies or renal insufficiency
  • prenisone or solumedrol (methylprenisolone) starting 24h prior
  • Benadryl IM 1 hour or IV 4 min) prior
  • +/- H2 antagonists

Radiocontrast-induced nephropathy

  • IV saline hydration 24hr before and after
  • 600 mg acetylcysteine PO BID 24h before and after exposure
  • fenoldopam (dopamine-1 receptor, dopamine-1 agonist)
 
MOA of NAC:
  • source of sulfhydryl groups →converted into metabolites capable of stimulating  glutathione  synthesis, promoting  detoxification,  and  acting  directly  as  free radical scavengers.
  • inhibits ACE, which has been shown to be involved in experimental RIN.
