Non-emergency allergy prep (24h)
- use for allergies or renal insufficiency
- prenisone or solumedrol (methylprenisolone) starting 24h prior
- Benadryl IM 1 hour or IV 4 min) prior
- +/- H2 antagonists

Radiocontrast-induced nephropathy
- IV saline hydration 24hr before and after
- 600 mg acetylcysteine PO BID 24h before and after exposure
- fenoldopam (dopamine-1 receptor, dopamine-1 agonist)
MOA of NAC:
- source of sulfhydryl groups →converted into metabolites capable of stimulating glutathione synthesis, promoting detoxification, and acting directly as free radical scavengers.
- inhibits ACE, which has been shown to be involved in experimental RIN.
